This class is being videotaped and photographed. The on-stage demonstrations will also be filmed. I understand that I can fully participate in the class without volunteering to be an on-stage demonstration subject. If I do come on stage to serve as a demonstration subject, I grant permission for my image and voice to appear in training programs produced by the instructors and their affiliates. I also realize I might appear in background audience shots, as well as other "still" photos taken by various Innersource staff members. I grant permission for my image to appear in any Innersource publicity and social marketing efforts.

Withdrawals 45 or more days before the start of class, will receive a full refund for tuition (including Review and Post Class) - minus an $80 administrative fee.

Withdrawals must be emailed to certification@innersource.net and will be promptly acknowledged once received.

If you do not notify us at all of your withdrawal (i.e. if you do not attend a class for which you registered), you will not receive any refund and you will also be responsible for all hotel charges that have been incurred in your name for that class.

In the case of emergencies (death or injury) or catastrophic (natural or accidental) events, you may contact Innersource to discuss your situation. In these instances, decisions will be made on a case–by–case basis.

* If you have paid for a class and then withdraw less than 45 days before the start of the class, you will receive a 50% refund for tuition (including Review and Post Class) - minus the cost of your entire hotel reservation that Innersource made for you if you made a Private hotel room reservation.

Innersource has to guarantee room reservations 45 days prior to the event and if you cancel, Innersource will still be charged for your hotel reservation and must then pass on that expense to you.

For our US classes, you are responsible for making your own hotel reservations, and for any/all costs and fees incurred.

For our UK classes, since we make your hotel reservations for you through our group account (and you pay for them at the hotel desk), the hotel information you provide on your registration form is considered your commitment to that hotel room and to any roommates you list.

Please email madisonking@hotmail.com if you need to change any information about your hotel reservation after you have submitted it to us on your registration form (cancelling, changing arrival or departure date, changing from a Twin occupancy room to a Single occupancy room or a different Twin occupancy room, etc.).

As a common courtesy, you are responsible for contacting your roommate if you plan to change anything about your room reservation.

Please be aware that there are a limited number of twin rooms available at Wotton House and we cannot guarantee one will be available for everyone.

If - less than 45 days before the start of class - you cancel your room, change your rooming preference from Single occupancy to Twin occupancy OR from Twin occupancy to Single occupancy, you are responsible for your ORIGINAL room rates based on your original reservation. Innersource has already guaranteed payment to the hotel on the room based on your original reservation. If you leave a Twin occupancy room, this now leaves your roommate in a Single occupancy room and the original payment matter will defer back to the roommates to negotiate.

Innersource Release Agreement
I understand that Energy Medicine is an innovative approach to health care, which is only beginning to be investigated scientifically. I further understand that the training in Energy Medicine and other services I am or may be receiving from Innersource do not substitute for diagnosis and/or treatment from a qualified health practitioner for illnesses, injuries, or other medical conditions.

I understand that some classes are videotaped, that these recordings may be distributed for educational purposes, and I give my permission for my participation to appear on these recordings. Video recording, without explicit permission from Innersource, is not allowed.

Innersource staff reserves the right to prevent anyone from attending a class as it deems appropriate, or to require anyone to leave a class, as it deems appropriate, refunding only the prorated portion of the tuition not used.

This class may use massage tables provided by Innersource for you to lay on as a partner works with you during some of the practice sessions. This is basically a safe and sound practice that has been employed in the past by us, as well as by many other groups that teach healing procedures.

To avoid any possible mishaps, it is ultimately your responsibility to insure that the table is set up correctly (that the legs are secured) and that the table is not damaged. Please be sure it can bear your weight by sitting on it first to test its ability to support you before committing yourself to lying down on it.

By submitting this form, you agree to assume full responsibility for your safety in using a massage table for practice sessions. You agree to take the precautions described above and will not hold Innersource, any of its employees, associates, or contractors, or any hotel Innersource works with, liable for any mishaps that might occur as a result of using massage tables in this manner

I also understand that any activity in which a person might participate involves some risk. In consideration of being permitted to enroll for the instructional and other services offered through Innersource, I release and hold Innersource, its agents, officers, employees, directors, successors, and affiliated corporations harmless, unless gross negligence can be demonstrated, from any claims arising out of my participation in the instructional or other services offered by Innersource or the use of facilities and/or equipment it provides.

We reserve the right to change the Seminar Presenter for illness, emergent, or unforeseen circumstances.
